IVF (in Vitro Fertilization)

The first test-tube baby was Portuguese Carlos Saleiro today academic footballer born in 1986.

The birth of the first child by a technique of medically assisted procreation resulted from In Vitro Fertilization treatment (IVF) carried out in England in 1978, Lesley Brown, a 30-year-old woman with the horns clogged and there are 9 years struggled to be a mother. The baby Louise has today almost 35 years and was, herself, the mother of a child born in 2006 of natural conception.

The first test-tube baby was Portuguese Carlos Saleiro today Academic footballer (having also previously represented Sporting and played dozens of times in the national teams of football at youth), who was born in 1986, as a result of IVF treatment carried out at Hospital de Santa Maria, in Lisbon. Currently, every year hundreds of thousands of IVF cycles all over the world, and this is a safe procedure, well established, with a great percentage of success and which has already helped to birth children milhoes.

In IVF egg cells are collected from the ovaries of the woman, being then fertilized with sperm in laboratory medium. The embryos thus obtained are then transferred to the uterus. So, this is a procedure in which fertilization takes place outside the woman’s body. The expressions “test-tube baby” or “in vitro fertilization” used in common language appear exactly because of the ova (s) be fertilized in the laboratory, in contrast to what happens naturally during breastfeeding according to Fashionruling.com.

What is in Vitro Fertilization

The realization of a cycle of IVF is a process that involves several complex steps. The woman begins by being subjected to treatment with medicines ovulation inducers (Gonal F, Menopur or Puregon, for example), administered by injection. Are these that will stimulate the ovaries to produce more eggs than usual.

The development of eggs is then controlled by performing periodic scans and blood tests (to determine the levels of some hormones associated with the process of development of the ova). When it appears that the follicles are already sufficiently developed and with high probability of containing in your interior a mature ovum, is given an injection of another hormone (human chorionic gonadotropin – hCG, whose trade name is Pregnyl or Ovitrelle), whose function is to induce the release of eggs from the ovaries.

At this stage the time has a fundamental role: LP (operation of collection of oocytes from the ovaries) should be held 35 to 36 hours after hCG administration. That is, it is very important that couples respect the hours indicated by the gynecologist for Administration of multiple injections, because a mistake at this level can undermine the whole process of treatment.

The LP is done with ultrasound control and consists of introduction into the vagina of a very fine needle, which will allow the collection of oocytes from each one of the ovaries. This operation is performed under sedation and lasts about 15 minutes.

On the same day, puncture will be asked the man to collect sperm at the clinic, so that it can be used in the procedure (although it is also possible to use previously frozen sperm). Once obtained, the sperm is centrifuged at high speed and subjected to a series of treatment processes, in order to select the strongest sperm and with better ability of fertilization.

After the puncture, the oocytes are transferred to culture media in the lab, and then brought into contact with the sperm for fertilization to occur, with formation of embryos. Once obtained, the embryos (usually only two) are transferred to the uterus of the woman, to deploy, and give rise to pregnancy.

The embryos are usually transferred between two to three days after fertilization. In cases where there are several good quality embryo transfer may be made to the fifth day, not having any extra charge couples with prolonged culture.

The supernumerary embryos that have not been used in the treatment and present conditions of viability, can be frozen and used in a cycle to be held subsequently (in cases where the couple plans to have a second child, or if the first attempt fails), can be donated for scientific research, can be donated to another couple or may be destroyed, and the couple the decision about your destination provided that the conditions laid down in the law.

From the day of the puncture, the woman begins to apply progesterone (vaginal tablets or gel) in order to prepare the endometrium for implantation of embryos can be successful.

What is in Vitro Fertilization

There is indication for IVF in the cases of:

-Unexplained infertility;

-Moderate to severe ovulatory Dysfunction;

-Obstruction of fallopian tubes (also in the case of women who made tubal ligation);

-Failures repeated in cycles of intrauterine insemination carried out previously;

-Infertility caused by male moderate severity factor because it is a treatment in which the process of ovulation and the probability of fertilization are optimized, as well as the likelihood of implantation in the womb of one or two embryos in vitro fertilization is an alternative with excellent chances of success and that allows you to solve the problem of a large number of cases of infertility.

Success rates of in Vitro Fertilization

The success rates of IVF are quite variable and mainly depends on two factors: the cause of infertility and the age of the woman. How much more complicated for the diagnosis of infertility and older for the woman, the lower the likelihood of successful treatment. In the case of men, the age won’t be as relevant, although the probability of success is greater in younger men.

In the most recent international reports the success rates for IVF cycles with fresh embryos (clinical pregnancy confirmed ecograficamente by embryo transfer) are about 30-35% and for cycles with frozen embryos are around the 20% (European Records of the ESHRE).

Although many times the rates of success of PMA treatments tend to be considered too low, we should not forget that in situations where there are natural conception the probability of success is the same: only 20 to 30% of the attempts result in pregnancy.

On the other hand, it is important that couples should take into account the probability of having a real son is slightly lower because, as happens in cases where the design if it makes natural mode, some women can’t take the pregnancy to term and risk abort spontaneously.
